Treatment Policies


Virtual consultations provide limited assessment and are appropriate only to treat non-acute conditions. Please follow up with your primary care provider or go to emergency services if you suspect you have an acute condition. We do not prescribe controlled substances except testosterone.

Health Care Intake Form Policies

Please fill out the appropriate intake forms, consents, and waivers prior to your appointment. These forms will be sent directly to your email upon confirmation of your appointment or can be accessed through the patient portal. All forms must be completed at least 48 hours prior to your appointment so we can provide the best care you deserve.

Payment for Health Care Services

Rejuvenate Women's Wellness is NOT currently affiliated with any health insurance networks, and is considered out of network. If necessary, we can assist in completing forms for FSA/HSA reimbursements or prior authorizations. We do not guarantee insurance coverage or reimbursements.  We accept major credit/debit cards. Once your payment is received, your appointment will be confirmed. Refunds are provided only if appointments are canceled more than 48 hours within your appointment time. If you have any questions or concerns, please contact us to resolve any issues.
